Renewed NCI Investigational Agent Accountability Record Forms (DARFs)
The Office of Management and Budget (OMB) has approved the NCI Investigational Agent Accountability Record (DARF) forms. No changes have been made to the forms. The forms with the new expiration date are posted to the CTEP website https://ctep.cancer.gov/forms/.

• NCI Investigational Agent Accountability Record Form
Replace unused saved or printed copies of expired DARF forms at your site. Sites must transition to use of the new forms within 90 days of the expiration date of the current form (i.e., within 90 days of March 31, 2019).
